8D report
Also called: Eight disciplines problem solving
A structured problem-solving report in eight steps, from containment to root cause to permanent corrective action, used with suppliers after a defect.
The eight disciplines (D1 to D8) cover forming a team, describing the problem, containing it, finding the root cause, choosing and verifying corrective actions, putting them in place, preventing recurrence and closing. Customers commonly set response windows, such as containment within a day or two and a full report within weeks.
An 8D should only close when the fix is shown to work.
Where it goes wrong
Containment gets done, then the root cause and permanent fix wait on emails between companies, and the same defect comes back.
What Spurby does
Keeps each 8D’s steps with owners and due dates from emails and calls, and chases the step that’s late.
